The Story

Born to Fight

Three days into fatherhood, Samuel del Real's world changed forever. What began as the happiest moment of his life became a race against time.

Three days ago, I became a dad. It was the most beautiful moment of my life. But my son was born with a rare birth defect within his heart and right now he's fighting for his life.

Open-Heart Surgery

Soren faces open-heart surgery — his only chance at a full life. The operation is complex, the costs are immense, and every day counts.

What is a Congenital Heart Defect?

A congenital heart defect (CHD) is a structural problem with the heart present at birth. Some defects are mild; others, like Soren's, are life-threatening and require open-heart surgery — a complex operation on an infant that carries enormous risk and cost. Without surgery, children with severe CHDs cannot survive.

Understanding the Condition

Tetralogy of Fallot

TOF is one of the most complex congenital heart defects a baby can be born with. It is not one problem — it is four, all at once. Understanding what Soren faces helps understand why your support matters so much.

What is Tetralogy of Fallot?

Tetralogy of Fallot (TOF) is a critical congenital heart defect — meaning it is present at birth and requires intervention in the first year of life. “Tetralogy” means four: TOF is actually a combination of four separate structural problems with the heart that occur together. Each one alone would be serious. Together, they drastically reduce the oxygen supply to a newborn's body.

Babies with untreated TOF develop a condition called “blue baby syndrome” — their skin, lips, and fingernails turn blue (cyanosis) because their blood carries too little oxygen. Without open-heart surgery, TOF is fatal.

01

Ventricular Septal Defect

VSD

A hole in the wall (septum) separating the left and right ventricles. Oxygen-rich and oxygen-poor blood mix, reducing the amount of oxygen delivered to the body.

02

Pulmonary Stenosis

PS

A narrowing of the pulmonary valve or the artery leading to the lungs, restricting blood flow from the heart to the lungs and making it harder to get oxygen.

03

Overriding Aorta

OA

The aorta — the heart's main artery — sits directly above the hole in the septum rather than only the left ventricle. This means it receives blood from both sides of the heart.

04

Right Ventricular Hypertrophy

RVH

Because the right ventricle has to pump harder against the narrowed pulmonary valve, its muscle wall thickens over time — weakening the heart progressively if left untreated.

Signs & Symptoms in Newborns

Blue or purple tint to skin, lips and fingernails (cyanosis)

Difficulty breathing or rapid breathing

Poor weight gain and difficulty feeding

Fainting or loss of consciousness

"Tet spells" — sudden episodes of deep blue skin during crying or feeding

Clubbing of fingers and toes over time

Heart murmur detectable at birth

Extreme fatigue and low energy

Open-Heart Surgery to Correct TOF

This is one of the most complex surgeries performed on a newborn. It typically lasts 4–8 hours and requires a specialised paediatric cardiac surgical team.

1

Heart-Lung Bypass

A bypass machine takes over the heart and lungs, keeping blood circulating while the surgeon works on the stopped heart.

2

VSD Patch

A Gore-Tex or pericardial patch is sewn over the hole between the ventricles, separating the two blood pathways.

3

Pulmonary Valve Repair

The narrowed pulmonary valve and outflow tract are widened — sometimes using a patch — to allow adequate blood flow to the lungs.

4

Aorta Repositioning

The aorta's position is addressed as the patch repairs redirect blood flow to ensure it only receives oxygen-rich blood from the left ventricle.

5

Recovery & Monitoring

The baby is transferred to a cardiac ICU for weeks of close monitoring. Most children go home within 2–4 weeks and can lead full, active lives.

Life After Surgery — The Good News

When caught early and treated by an experienced surgical team, TOF has one of the best outcomes of all critical congenital heart defects. The vast majority of children who undergo complete repair go on to live full, active, and healthy lives.

Why the Financial Burden Is So Heavy

Open-heart surgery for TOF in a newborn is not just expensive — it is one of the most resource-intensive medical procedures that exists. Costs include:

Paediatric cardiac surgery team fees

Heart-lung bypass machine operation

Weeks in a cardiac intensive care unit (CICU)

Post-operative monitoring and medications

Follow-up surgeries in later childhood (often needed)

Travel and accommodation for the family

Long-term cardiology follow-up care

Lost income for parents during hospitalisation

Total costs can exceed $150,000–$300,000 USD in the United States — and that is before follow-up procedures. This is why Samuel turned to the community: the financial weight of saving Soren's life is enormous.
